Home Clinic
IntefleCS Home Clinic has been designed to support new and emerging models of care which enable patients who would formerly have continued their treatment in hospital to be treated instead in the comfort of their own home.
IntefleCS Home Clinic is about delivering complex care safely outside of a hospital setting so that patients can be discharged from hospital sooner than they would otherwise have been and supported at home to ensure that they remain well and avoid further admissions. It will help you lead the change to a more cost effective but high quality model of care which patients really want and benefit from.
